1. A A new certificate must be issued before you sail.
2. B The existing certificate is endorsed as valid for a five year period commencing 27 May 1988.
3. C The existing certificate is extended for a period of up to 150 days.
4. D The existing certificate is extended until the first foreign port of call where a new certificate will be issued by the local surveyor.
