1. A A Certificate of Discharge Form should be attached to the book.
2. B An entry should be made in the book and a Certificate of Discharge Form issued to the seaman.
3. C If a vessel was on coastwise articles, the record of discharge will be made in the Official Logbook.
4. D The record of entry in the book must be submitted to the Coast Guard.
