How is this "International Only", if you are pulling out of "A Slip in the Harbor"??

This question means vessel has to sound for making way in RV as fog signal( for international). But question has trick that vessel slip in a harbour need to sound one prolonged blast (for inland only).

Only 3 short if operating astern propulsion WITHIN SIGHT of another vessel (Rule 34). One prolonged for Res Vis is needed. Within Res Vis, probably not going to be able to see a vessel unless it's mentioned.
