The rules specifically state to alter course to Starboard, I believe this should be removed so no one gets confused.

Don’t take the bait on this question! Ignore the diagram, and note that this is an INTERNATIONAL situation. What if the question just asked “what does two toots mean”? The answer would still be “altering course to port”. You are correct in that Rule 14 INTERNATIONAL says to alter course to starboard. Interesting that the INLAND version of Rule 14 says “unless otherwise agreed”, alter course to starboard. The bottom line is that right or wrong, in this situation in international rules, a meeting vessel sounding two toots means altering course to port.
