Technically, shouldn't this question be Inland only, given that the overtaking vessel is sounding an 'intention' rather than telling you he is doing it?

I believe anytime you feel that a vessels actions are unclear or may cause danger, 5 short and rapid blast are in order.
Colregs states that a vessel overtaking in a narrow channel must sound his intentions (PPS or PPSS) and the overtaken vessel must reply with PSPS (Charlie) if she agrees. If she does not (her answer is NO), she must sound 5 short and rapid blasts in response.
