Four short blasts... no prolonged blast... says it right in the book this question is bad.

It should have none of the above. " A pilot vessel engaged on pilotage duty" should sound 4 short blasts with no proceeding prolonged blast

This question is not wrong... see rule 35... "(k) A pilot vessel when engaged on pilotage duty may, in addition to the signals prescribed in Rule 35(a), (b) or (g), sound an identity signal consisting of four short blasts."

"(a) A power-driven vessel making way through the water shall sound at intervals of not more than 2 minutes one prolonged blast."

Think of it this way: NUC, RAM, Fish, Sail.. they all sound PSS. The P proceeds the SS. So for a vessel engaged in Pilotage it makes sense that the P proceeds the SSSS.
